The Lafayette Cancer Coalition would like to recognize and honor those who have survived cancer and those who lost their fight with cancer. To do this, we have constructed a portable wall to hang pictures and names of those courageous people. To add your loved one(s) to this wall each year, please fill out the form below. The cost is $25.00 yearly and the form contains further information on the type of display. You may send your money to LCCC, PO Box 88, Higginsville, MO 64037 and email your picture to lafcocancer@gmail.com. This wall will be displayed at the annual golf tournament and annual walk-a-thon plus other cancer events.
Each year you will need to fill out this form for your loved one’s picture or name to re-appear. For further information or questions, please contact any board member or volunteer of LCCC.
